Registration

Patient Information 

To register at our GP Practice you must complete a registration form known as a HS-200 (Persons transferring from one Northern Ireland or GB Practice to another Practice within Northern Irelan or for eligible visitors or persons taking up ordinary residence in Northern Ireland to complete HSCR1 form, which can be obtained from the surgery). Proof of ID and address will be required when registering. Further information can be found here When you register with our surgery, you will be invited to have a health check. Health checks are typically done by our healthcare assistants who will ask you about your personal and family medical history. Where possible, please bring a copy of your medication history to this appointment.

All patients registering with the Practice will be required to register for Online Patient Services.

Newborn Babies

Parents of a newborn who have been given a yellow infant registration form should still complete and submit the form to the practice.  The practice will still register newborn babies of either parent who is registered with the practice.
