If required, our GPs can complete the Private Medical forms, including the following:
- DVA
- PIP
- Insurance Questionnaires
- Universal Credit
- Travel Cancellation
- BUPA
- Blue Badge
- Child Minding
- Bin Collection
*Its important for patients to note, that whilst we are willing to accept these forms and complete the work, we cannot offer any estimated turnaround time. This is because the completion of Private Medical forms is not part of the GP contract. If the forms need completed by a specific date, then we can provide you with a brief summary printout which can be brought to a private provider for completion*
